We are in the medical field, so we have access to tons of blank CDC Covid 19 Vaccination cards. Most importantly, we have a full access to the database of Pfizer and Moderna vaccine lot numbers.

Physical card will be shredded after picture is taken. We do not mail the card itself due to the high risk involved for both us and yourself. Besides, no one will ever ask you for it anyway. A picture is what is needed as a proof of vaccination.

Every card will have 3 vaccine lot numbers that will match your chosen vaccination location. There is a vaccine lot database that has numbers but not names, so your name will not show up and cannot be verified.  We provide lot numbers for Pfizer and Moderna.

3.  Two dates that are approximately 30 days apart, in addition to a third date for your Booster shot, that you wish to be your vaccination dates.

FAQ
Q1.  Can my employer find out that this card is not real
A1.  No. Even if your employer decides to check, they will find the lot numbers to be real and matching your alleged vaccination location and date.
Q2.  I heard in the news that some people got caught presenting fake cards.
A2.  Most likely their lot numbers did not match. You cannot just make those up.
They have to be 100 percent correct from the vaccine database and will have to match not only your alleged vaccination location, but also the date.
Q3.  Seems that I am paying for a card that I can download myself and fill out.
A3.  Unless you know the correct lot numbers, we do not advise you to do that.
Your payment basically goes towards the accurate lot numbers that match your alleged vaccination location and date. Paper card itself is worth nothing.
Q4.  What if my employer asks for the real card itself
A4.  They will not. They simply follow their rules by keeping a copy of your card on file. 
It will be impossible for them to ask everyone to show their physical card first before taking a picture and most people misplace their actual cards.
